In order to better organize and streamline the holiday service, we have enabled this form to take your requests for prayer for health in advance, so that you do not have to stand on line during the service and wait to say the names of people to the Rabbi.

The names you submit below will be mentioned on Rosh Hashanah and Yom Kippur during the Torah reading in a special blessing made for people who need special prayers for health.

It is customary (not required) to make a donation in honor of the people receiving this blessing for health.
The text of the prayer for health mentions that charity was given in this person's honor.
You may give any amount, either specific for each person or one total amount for all.
