AGREEMENT
I understand that participating in group outings during Covid involves risk, I assume this risk on my behalf, and all those I am registering assume this risk as well. I, and all registrants release Bris Avrohom of Fair Lawn from any liability.

Requirements for attending:
I and everyone I am registering have not been in close contact with anyone diagnosed with Covid in the 14 days leading up to this event. (March 15 - 28)

In the 14 days leading up to this event, I and the people I am registering, have not been diagnosed with Covid-19, and have not experienced any of the following symptoms:
• Temperature of 100.4 or higher.
• Cough associated with a cold
• Shortness of breath
• Chills
• Muscle pain
• Sore throat
• Loss of taste or smell
 
I agree that I, or anyone I am registering will not attend this event if the requirements above cannot be met. 

Refund Policy:
I understand that if I inform Bris Avrohom of Fair Lawn via email at least two days before the event, or asap when I realize we cannot attend, I will receive a full refund for the individuals that cannot attend. I further understand that if I cannot attend, I may ask via email, that my payment be kept as a donation, and I will receive a tax deduction for the amount.
