BAT MITZVAH CLUB AGREEMENT

I agree that my/our child/ren be in photos & video and allow these to be published in any form.
I agree that my child participate in the Bat Mitzvah Club program and trips and assume all responsibility and risk on my behalf and for my child.
I understand that participating in the program and group trips involves a degree of risk, both anticipated and not anticipated.
I agree to release Bris Avrohom of Fair Lawn from any liability.
I understand that participating in group outings involves risk of infection of any kind, and I assume this risk and release Bris Avrohom of Fair Lawn from all liability.
In order to ensure complete safety, children participating in Bat Mitzvah Club must remain within the group at all times, unless instructed otherwise by an authorized Bat Mitzvah Club leader/coordinator.
A pleasant atmosphere and proper decorum is integral at all Bat Mitzvah Club meetings, trips and events, and thus, any conduct or language that expresses disrespect for others will not be tolerated, and the individual/s will be asked to leave.

Medical Release
In case of an emergency, I request that the Bat Mitzvah Club leader contact me. I hereby consent to the administration of Bat Mitzvah Club and Bris Avrohom of Fair Lawn to take whatever medical measures they deem necessary for my child in the event of a medical emergency.
